Family Court: Case Data Collection

Here it is, the form and data collection you have been asking for to share the relevant names and basic facts of your case. The form is respectful of your case and is specifically designed to collect data on your experience with the family court and family legal service systems of New Hampshire. Fill out the form in full except where it indicates (optional).


If you have had more than one judge, more than one court, more than one GAL, or more than one attorney, etc., name each one to properly document their names and the number of judges, etc. Example:( Referee James Smith, Marital Master John Doe, Judge Sara Doe, Hearing Officer Jon Smith) Up to 250 letters. If you need more space, submit a separate form.


If something does not apply please write N/A or skip the question if it is not required (*). Please read thoroughly before you begin to familiarize yourself with the data. All Data will be placed into NHFJ public data collection spreadsheet about the families 'real-life experiences' by the states family legal systems.
